This facility enables user to request assistance for the following:

1. Assign as School Head:

  • This is for reassigned or newly promoted school heads. Before requesting, please make sure that ALL ROLES IN LIS were REMOVED. System does not allow assignment in EBEIS of accounts with School Personnel Role.

2. Assign as School System Administrator:

  • This is for transferred or newly designated School System Admin (School ICT Coordinators).
  • NOTE: School Head can also assign school system administrator.

3. Reset Password of School Head of School System Admin:

  • This is for those who forgot their password in LIS.
  • NOTE: Password Reset can also be done at school level by the school head or system admin.
  • ANOTHER NOTE: Temporary Default Password is the USERNAME.

NOTE: Reference Number of Latest Accepted Report in DUDGT 4.0 Information of School Head and School ICT shall be REQUIRED.
